0:40 from Siegward of Catarina

Grand Archives:
0:50 hidden behind the bookcase a chest containing a Slab
1:10 after killing all Golden Winged Knights at the very top of Lothric Castle
1:30 found after activate the elevator outside the Lothric Princes boss

Archdragon Peak:
1:52 next to Havel Knight
2:04 after defeating Nameless King

[DLC] Painted World of Ariandel:
2:14 near the Depths of the Painting bonfire
2:56 after defeating Sister Friede second phase
3:03 from Corvian Crow after defeating Sister Friede
